Population Health

THE CHALLENGE

Despite numerous marketing campaigns and population outreach, Baptist Hospital was facing decreasing ED and inpatient volume due to patient migration to larger urban hospitals which had more outpatient capabilities.

THE SOLUTION

RHS was asked to evaluate strategic opportunities to increase volumes. RHS recommended the introduction of population health programs to increase cancer, disease, and immunization management with the population that had used Baptist Hospital’s services over the last 3 years.

With RHS leadership, Baptist Hospital developed a program to enroll their population in the program and then led the organization through the rapid development of their Chronic Care Management and Annual Wellness Visit programs, increasing support and evaluation of their highest risk patients in better caring for chronic diseases.

THE RESULTS

With the introduction of provider training programs, the introduction of an RHS Population Health Nurse Director, and supporting technology systems, an organized outreach program focused on improving the health of the community Baptist Hospital was able to increase their outpatient volume by 20%, improving their organization to profitability once again. CEO John Andrews said, “although we are not finished increasing our services to our community, without the help of RHS, we would still be looking for a meaningful solution to provide better care. We love our Population Health program.”
